The 5-minute version

  • Same-level falls injure more workers than almost anything else in industry — and they’re the most preventable injuries on the books.
  • In our world, the slip hazard has a name: spillage. Carryback piles, fines on walkways, washdown water. Housekeeping is safety.

Why this talk exists

Slips, trips and falls on the same level don’t make dramatic headlines, but they fill emergency rooms: they’re consistently among the leading causes of workplace injury and workers’-comp cost in national data, and OSHA’s walking-working-surfaces rules (1910.22) exist because of them. In a bulk-handling plant the pattern is specific: material on the floor came off a conveyor, out of a chute, or through a failed seal — which means every slip hazard is also a maintenance finding.

The rules that matter

  • Clean, orderly, dry — that’s the actual regulation. 1910.22 requires walking surfaces kept free of hazards: spillage, water, grease, loose boards, obstructions.
  • Fix the source, then sweep. A pile under the head pulley is a belt-cleaner work order wearing a housekeeping costume. Sweeping it daily and fixing nothing is scheduling the next slip.
  • Footwear matters and so does pace. Slip-resistant soles for wet process areas; and the plant walk is a walk — the incident reports are full of people who were hurrying.
  • Light the route. Burned-out lamps over stairs and along conveyor galleries get fixed like the hazard they are.
  • Winter and washdown discipline: ice, fog-system overspray and hose water get controlled, signed, or dried — not narrated (“careful, it’s slick over there”).

How it happens — a scenario

Based on the kind of incident that happens every year

The secondary cleaner on the main line has been due for blades for a month. Carryback drops along the return run, and the crew has quietly adjusted: everyone knows you walk the far side of the gallery because the near side is always covered in fines.

A contractor doesn’t know. Carrying a ladder to the head end, he takes the near walkway, hits the layer of dry fines on smooth checker plate, and goes down hard with the ladder on top of him. Wrist fracture, eight weeks, recordable.

The investigation finds what everyone already knew: the walkway had been hazardous for weeks, normalized by the people who’d learned to avoid it. The blade change that followed took forty minutes. The fines stopped. So did the hazard nobody had reported because “that’s just how that walkway is.”

Talk it through

Five minutes of reading is worth less than two minutes of arguing. Ask the crew:

  • Which walkways here do we 'just know' to be careful on — and what’s the upstream fix for each?
  • Where did the last three slip/trip near-misses happen? Did anyone write them up?
  • What’s living in our walkways right now that shouldn’t be? (Hoses, cords, pallets, parts.)
  • Whose job is the leak vs. whose job is the puddle — and does the leak ever actually get reported?

Are slips and trips really worth a talk versus the big hazards?

By injury count and cost, they ARE a big hazard — among the largest. They just fail quietly, one wrist and one knee at a time, instead of all at once.

What's the single highest-value fix in a bulk plant?

Stopping fugitive material at its source: belt cleaners doing their job, sealed transfer points, fixed leaks. Plants that fix carryback watch their slip numbers fall with it.

Grating or checker plate for conveyor walkways?

Open grating sheds fines and drains water, which is why galleries use it. Where smooth plate exists in a material path, it needs aggressive housekeeping or a surface upgrade.
