Toolbox Talk: Lockout / Tagout

Five minutes on the padlock that outranks every schedule in the plant. Print it, read it at the tailgate, argue about it.

The 5-minute version

  • If it can move, kill it and lock it before your hands go anywhere near it. Every energy source, every time.
  • Your lock, your key, your life. Nobody else's lock protects you — and nobody removes yours but you.
  • Stored energy counts: a stopped conveyor with a loaded incline, a charged air tank, a raised counterweight — all still armed.
  • Try to start it after locking out. Verification is the step that catches the wrong breaker.
  • Lockout violations are OSHA’s #4 most-cited standard — and the reason is always the same: “it was just going to take a second.”

Why this talk exists

OSHA estimates that proper energy control prevents on the order of a hundred-plus deaths and tens of thousands of injuries a year — and lockout/tagout still sat at #4 on the most-cited list in FY2025 with over 2,100 violations. In our industry the machine in question is usually a conveyor, a screw, a rotary valve, or a flow aid plumbed to plant air. Every one of them has ended careers — almost always during cleaning, unjamming, or “a quick adjustment.”

The rules that matter

  • One energy source is never the whole story. A conveyor has electrical, gravity (the loaded belt), tension (the take-up), and sometimes pneumatic all at once. The lockout isn’t done until all of them are.
  • Personal locks, personally applied. A group lockbox works because every worker’s own lock is on it. “The supervisor locked it” protects the supervisor.
  • Verify by attempting a start. Push the start button after locking out. If something spins, you just learned it the survivable way.
  • Air cannons get locked out too. Isolate and bleed the tank before anyone works near the nozzle path — a cannon that fires during maintenance is a cannon-sized problem.
  • Releasing the lockout is a procedure, not a shout: tools clear, guards back, people accounted for, then locks off — each by its owner.

How it happens — a scenario

Based on the kind of incident that happens every year

Second shift, a plugged transfer chute. The crew shuts the conveyor off at the local disconnect — but nobody locks it, because the poke bar usually clears it in five minutes and the disconnect is right there.

Twenty feet away and out of sight, the control room sees the “stopped” flag on the screen, assumes the jam cleared itself, and restarts the line remotely. The belt lurches as the man is leaning into the chute with the bar.

He lost the bar and two fingers, and kept his life only because the belt was loaded and slow to ramp. The disconnect had a hasp on it. His lock was in his box, thirty steps away. The distance between that incident and a fatality was luck — and the distance between it and a non-event was one padlock.

Talk it through

Five minutes of reading is worth less than two minutes of arguing. Ask the crew:

  • Which machines here have more than one energy source — and can everyone name all of them?
  • Where does stored energy hide on our conveyors? (Take-ups, inclines, counterweights, air receivers.)
  • When did each of us last actually push start to verify a lockout?
  • Is there any job we routinely do “real quick” without locking out? Why?

Frequently asked questions

Does a stopped machine really need lockout?

If your body enters the danger zone, yes. 'Stopped' means the machine is between movements; 'locked out and verified' means it can't move. The gap between those two is where the injuries live.

What about tasks too small to lock out?

OSHA's minor-servicing exception is narrow and requires equivalent protection. The honest test: if a surprise start would hurt you, it's not minor.

Who removes a lock if its owner went home?

Only through a formal employer procedure: verify the owner is absent, make every effort to contact them, and document it. Bolt cutters on a whim are how the next shift learns the wrong lesson.
