Roughly 90-plus workers die in confined spaces every year in the U.S., about one incident in five kills more than one person, and the multi-fatality pattern is always the same: someone goes down, and the people who love them or work with them go in after — unprotected, into the same atmosphere or the same material. Around 60% of confined-space victims are rescuers. In bulk handling the classic killers are oxygen-deficient headspaces, CO from combustion or self-heating material, and engulfment in flowing or collapsing bulk solids.
Based on the kind of incident that happens every year
A cement plant’s silo is bridged. Production is hurting, and the crew decides to handle it themselves: drop a ladder through the top manway, send the newest man down onto the material with a bar to “just poke it loose.”
The bridge he is standing on is the blockage. It lets go under him exactly the way bridges do — all at once — and he goes down with the material into the funnel of a vessel that was never isolated. His supervisor starts in after him and is stopped, barely, by the third man.
This scenario is reconstructed from a pattern MSHA and OSHA fatality reports repeat year after year, almost word for word. It is the single reason our silo crews work from the top deck with the tooling inside and the people outside — and why any contractor whose plan involves a man on the material should be shown the gate.

Typically yes — limited entry, not designed for continuous occupancy, and potentially hazardous atmosphere and moving parts. Bag change-outs get planned accordingly.
A harness does not defeat engulfment or atmosphere — it just changes what the rescue team recovers. Professional cleaning works from outside the vessel precisely so this question never needs a better answer.
Annually at minimum, and any time a process changes. New ductwork, new material, or a new vessel can turn yesterday's non-permit space into today's permit space.
